That's a basic boutique bow. I can't really help you - I use No Bo No Go templates for mine.

The tails are in the back. I also use NBNG templates.

Those are really cute bows..she's and HG too. I have NBNG templates and have also purchased a ton of tutorials on etsy..they have some really good ones out there.
