Order form

Does anyone have a customer order form they would be willing to share? Please. I'm at a loss as to what to include when making ours up. I am looking for something for like custom orders and such.

I'm not sure what all you sell but you'd need order date, promised completion date, customer contact info, method of payment, date of payment, item name or #, size, type of hardware, color, style, price, pick-up or delivery. Probably more stuff too. It varies with what type of items you create.
