I am guessing that they either made the center kind of like you would make a korker, with various lengths of decoration sewn together at a center point or by using the regular knot center as an attachment. For the first way, you could then just glue that korker type bundle to the center of the bow that is the main bow.
I would also imagine that you could attach it by using the knot you would normally use at the center of the bow as the attachment and the knot would be hidden by the decorations. You would just wrap the ends around the back and attach them like you normally do to finish a bow.
Since we cannot see the center its hard to tell which way she used. Both would work though.
